Accepted Insurance Plans

Navigating insurance coverage can feel overwhelming, but we’re here to make it simple. Whether you’re coming in for a routine check-up, diagnostic testing, or a specialty visit, our goal is to ensure you feel informed and confident about what’s covered.

At Centerville Clinics, we proudly accept most major health insurance plans, including Medicare and Medicaid. Our experienced staff is familiar with a wide variety of plans and will work with you to make your experience as smooth as possible.

Our Sliding Fee Program

Centerville Clinics welcomes all patients, regardless of their ability to pay. As a federally funded health care program, we’re able to offer a wide range of services through our sliding fee scale.

To qualify, you MUST provide proof of your gross annual income, which includes income from ALL immediate family members living in your household. Gross income means ALL income from ALL sources, before taxes are taken out.

Acceptable Income Verification:

  • Three (3) consecutive pay stubs or 1099
  • Previous year’s federal tax return or W-2 form
  • Proof of any form(s) of income not reportable on an IRS tax return

Good Faith Estimates

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

Under the law, healthcare providers must provide patients who don’t have insurance or are not using insurance with an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
  • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

Healthcare Insurance Assistance

Centerville Clinics, Inc. has Enrollment Assisters who can assist with applications for coverage through Pennie, Pennsylvania’s ACA marketplace. Assisters help with applications and discuss differences between all available plans, and answer all questions regarding Pennie plans.

Assisters also assist with initial applications and/or renewals of State programs, including MAWD, MA, CHIP, PH-95, and Medicaid Expansion.

Private one-on-one appointments are available at 11 offices in the tri-county service area. Individuals will choose the office that is most convenient, and all assistance is free of charge.
